community we serve is very important to the Oklahoma City Police Department.
If you do not know the name of the employee(s) involved, please provide
details such as the location, date, and time of the incident as well as your
contact information.
There are several ways you
can contact the Oklahoma City Police
Department with your feedback.
If you would like to
recognize a job well done by a member of the Oklahoma City Police
Department:

the involved employee(s) contact you.

supervisor to respond to your situation in a timely manner.

If you want to file a
formal written complaint:

You can file a formal
written complaint on the employee(s) by completing a Formal Citizen
Complaint Form. This form is available at any police facility, City
Hall or you can download and print the form by clicking
here. This form must be completed
and signed by the complainant. This form must also be signed by a
witness or notary public.
